Please send your grant in a 3-ring binder.

A digital copy of the grant request is appreciated.

 

STEPS:

  1. Contact Information: Name, Phone, Email, and Address
  2. Demonstration of need: Requested value and utilization of funds are clearly stated
  3. Specific plans and timetable of the project
  4. Description of funding plans: both current and long-term
  5. Objectives and background
  6. Mission Statement
  7. A current year-to-date financial statement
  8. The most current FORM 990 filed with the IRS
  9. Budget, including sources of funding
  10. List of Board of Directors and key personnel
  11. IRS 501(c)(3) tax determination letter

ALL ABOVE INFORMATION MUST BE RECEIVED NO LATER THAN AUGUST 31 IN ORDER FOR YOUR ORGANIZATION TO BE CONSIDERED FOR A CONTRIBUTION. CONTRIBUTIONS ARE MADE ANNUALLY IN DECEMBER.
